Use this form to report incidents related to distressed, aberrant, or disruptive student behavior. Examples of distressed or aberrant student behavior include: acting out of class, expressing mental health concerns, being excessively confrontational, exhibiting unusual or bizarre behavior, etc. A Student Intervention Team referral form should be completed within 24 hours of an incident and submitted online or to Anne Daly Eimer in Enrollment Services.
